Enrollment Procedure SY 2021-2022
Step 1: Register Online

The Appointment Date is the Date of Registration/ Enrollment
Download and print the Registration Form by clicking Print Registration Form.
Fill out the necessary information on the Registration Form and the Data Privacy Consent Form. (Students do not need to sign on the Data Privacy Form.)
Step 2: Payment
Online: Applicable only to those who have Metrobank and PNB account with activated online banking

Over the Counter Payment (At any branch of Metrobank and PNB), kindly use Payment Slip Form. Please see the sample below:

PAYMENT THROUGH A PNB BRANCH
1. Go to the nearest Philippine National Bank (PNB) branch and ask for a Bills Payment Slip.
2. Fill out three copies of the slip completely and legibly with the following information:

- Account name: Saint Jude Catholic School
- Reference number: your student number
- Subscriber name: your complete name

3. Print your name (again), the purpose of payment (e.g. tuition fee).
4. Make the deposit and get two copies of the validated slip.
5. Attach one copy of the validated slip to your documents. This will serve as your proof of payment.

How can I pay my PNB bill online?
1. Click Pay Bills on the menu.
2. Choose the source account for your payment.
3. Select your biller.
4. Enter the amount and reference number.
5. Confirm your payment details.
6. Enter the 6-digit One-Time PIN (OTP) sent to your registered mobile number.
Step 3: Fill out the Google form on this link:
For parents with 2 or more children, please use each of your child’s SJCS Gmail account in filling out the Enrollment Form. Please use this guide to switch account: (Google Chrome browser was used in the demo.)

Step 4: Wait for the confirmation and the Official Receipt to be sent to your child’s SJCS Gmail Account.
Sending of receipt will commence on June 14, 2021.
